VSP Eye Vouchers
If your student does not have any type of vision insurance coverage and you feel they need an exam, please come see me or contact me for VSP Eye Vouchers. 

 

Incoming 8th Grade & 12th Grade students for the 2016-2017 School year:
8th grade students must have a Tdap & Meningococcal vaccine prior to enrolling. 12th grade students must have had a Meningococcal vaccine on or after their 16th birthday, prior to enrolling.  You can get these at your local Heath Department or from a health care provider. Please bring documentation of these to the school as soon as possible.

 
Incoming Kindergarten students for the 2016-2017 school year:
You are required to have your Kindergarten set of shots prior to entry into Kindergarten.  
You can get these at your local Heath Department or from a health care provider.  Please bring documentation of these to the school as soon as possible.
 
 
Preventive Services Program  (PSP)
This is a state funded project, where each student is screened by a dentist, then fluoride varnish is applied to the teeth(if their parent has consented for them to receive this service).  We had 180 students participate last year!!  Dr. Fallon Stiens DDS and her wonderful staff from the Stanberry Dental office have commented that they can see improvement in our children's dental health. This is a huge compliment to our children, their families, and to this program that we can see visible positive responses!  A huge THANK YOU to Dr. Fallon Stiens DDS and her awesome staff for all their help!
